1. The project is a proposed expansion of the tumor treatment facilities of the Zibo Wanjie Tumor Hospital, located in an economic development zone in Zibo City, Shandong Province. This hospital is co-located with the Zibo Wan Jie General Hospital, these being two of several hospitals owned and/or managed by the Wan Jie Group. The current tumor treatment facility is 300 beds and 6 floors, to be expanded with the addition of 600 beds and 21 floors in a new facility. The facility will also include a new Proton Therapy System for diagnosis and treatment of cancer.

2. This is a Category B project, according to IFC’s Procedure for Environmental and Social Review of Projects because a limited number of specific environmental impacts may result which can be avoided or mitigated by adhering to generally recognized performance standards, guidelines or design criteria. The review of this project consisted of appraising technical and environmental information submitted by the project sponsor. The following potential environmental, health and safety impacts of the project were analyzed:
· handling and disposal of medical and bio-hazardous waste;
· liquid waste and sewage disposal;
· power and water supply;
· hygiene and implementation of good hospital management practices;
· employee health and safety training; and
· fire protection
The sponsor has presented plans to address these impacts to ensure that the proposed project will comply with applicable host country laws and regulations and World Bank Group requirements. The information about how these potential impacts will be addressed by the sponsor is summarized in the paragraphs that follow.

3. The expanded tumor treatment facility is located on land adjacent to the existing Zibo Wan Jie General Hospital, with which it shares services. No new land is being acquired and no relocation of people is required. The site is adjacent to other establishments owned by the Wan Jie Group.

4. Proper handling of medical waste is the most important environmental issue facing hospitals. In the Zibo hospitals, infectious and pathogenic medical wastes are segregated and burned in the hospital’s medical waste incinerator. The incinerator operates at a temperature of 800 to 1100 oC, and emissions meet the World Bank Group’s waste management guidelines for incineration systems. Radioactive wastes from the chemotherapy unit include a number of short-lived isotopes (13N, 18F, 99Tc and 125I). Wastes with these isotopes are isolated in lead containers for over 10 half-lives, allowing them to decay to negligible levels before discharge, as per standard international practice.

5. Liquid effluents, including sanitary wastes and wash water, are discharged to the municipal sewage system for treatment. Other general paper and plastics are handled by the municipal waste handling center.

6. Electrical power is supplied by both the Zibo municipal generating plant as well as the Wan Jie generating station located nearby. A standby generator is provided on site for the fire-fighting system.

7. Housekeeping in the hospitals requires improvement to meet international standards for safe operating conditions. The sponsor has committed to implementing, in conjunction with their international technical partner, a comprehensive upgrade of all aspects of hospital operations to ensure adherence to international best practice in hospital management. This will include, as per World Bank Group guidelines for hospitals, the development and implementation of a comprehensive waste management plan, incorporating detailed procedures for handling all types of wastes. The sponsor will ensure that hospital staff receive comprehensive training in health and safety matters and in fire safety and emergency response.

8. An independent fire and life safety audit of both the Zibo Wan Jie Tumor Hospital and the Zibo Wan Jie General Hospital indicated that substantial upgrading would be required to meet World Bank Group guidelines and NFPA standards. Major items needing attention include installation of smoke barriers, upgrading of doors (for fire rating), staircase enclosures, improved wiring and emergency lighting, improved hydrants and sprinkler system (in Zibo Wan Jie General Hospital) and installation of alarm systems, among others. The sponsor has committed to upgrading these facilities to international standard, and will prepare an action plan for implementation, prior to disbursement of this investment.

9. Accordingly, IFC concludes that the proposed project will be designed and implemented to meet the applicable World Bank Group’s environmental and social policies and the environmental, health and safety guidelines.

10. IFC will evaluate the project''s compliance with the applicable environmental and social requirements during the lifetime of the project by:
· Reviewing the annual monitoring reports (AMRs) prepared for the project, covering ongoing performance of project-specific environmental, health and safety activities as reflected in the results of periodic and quantitative sampling and measuring programs; and
· Conducting periodic site supervision visits.

IFC supports its clients in addressing environmental and social issues arising from their business activities by requiring its real sector clients to set up and administer appropriate grievance mechanisms and/or procedures to address complaints from Affected Communities in relation to environmental and social issues arising from IFC's clients' business activities. Since 2012, IFC's Financial Intermediary clients applying the Performance Standards are required to develop External Communications Mechanisms to receive and review inquiries or complaints from any interested party regarding the E&S risks and impacts of their operations.

For IFC projects that are active or under consideration, individual(s), or their authorized representative(s), who allege they have been or will be adversely affected by E&S risks or impacts arising from IFC's clients' business activities, may submit a complaint to IFC directly for consideration. When receiving an E&S complaint, IFC engages with complainants and works with its clients to find solutions to E&S concerns as quickly and efficiently as possible.

IFC complaint response is separate from and complementary to IFC’s clients’ project level grievance mechanisms and IFC’s Independent Accountability Mechanism (CAO). Lodging a complaint directly with IFC does not, at any time, restrict access to CAO.

Affected communities have unrestricted access to the Compliance Advisor Ombudsman (CAO), the independent accountability mechanism for IFC and MIGA. CAO facilitates the resolution of complaints related to IFC and MIGA projects and sub-projects, undertakes reviews of IFC’s and MIGA’s environmental and social compliance, fosters public accountability for their commitments, and enhances the environmental and social performance of IFC and MIGA.

Independent of IFC and MIGA management and reporting directly to the IFC and MIGA Boards of Executive Directors, CAO helps resolve issues raised about the environmental and social impacts of projects through a neutral, collaborative, problem-solving approach via its Dispute Resolution function. It also reviews IFC’s and MIGA’s compliance with environmental and social policies, assesses related harm, and recommends remedial actions through its Compliance function. Additionally, CAO provides advice to IFC, MIGA, and their Boards to enhance environmental and social sustainability performance and minimize the risk of harm.

Complaints may relate to environmental and social harm regarding any aspect of the planning, implementation, or impact of a project or sub-project, and can be made by any individual, group, community, entity, or other party affected or likely to be affected by the environmental or social impacts of an IFC- or MIGA-financed business activity.
