ENVIRONMENTAL REVIEW SUMMARY (ERS)



Project Name : Côte d’Ivoire – AEF Simmeq

1. The project is for the construction and operation of a 79 bed private hospital in Abidjan, the capital city of Côte d’Ivoire. The hospital, “Polyclinique les Graces”, is being built in Zone 4, a residential neighborhood in southern Abidjan. The clinic is located on a 6,975 square meter plot of land owned by the main project sponsor, Dr. Aristide Zepka. Construction work started in 1998 and “Polyclinique les Graces”, is scheduled to begin operation in October 2000. It will offer specialized services in gynecology and obstetrics, cardiology (consultations and basic treatment), general medicine, nephology with an hemodialysis center, cancerology (consultations and basic treatment) and pain relief center, emergency treatment and general surgery, an intensive care unit and a medical transport unit. The clinic will also include a medical scanning and diagnosis center, a medical laboratory and a dental office.

This is a category B project according to IFC’s environmental review procedure because specific impacts may result which can be avoided or mitigated by adhering to generally recognized performance standards, guidelines or design criteria. The review of this project consisted of the appraisal of technical and environmental information submitted by the project sponsor and subsequent verbal discussions. The key environmental, health and safety issues considered in this project include :

§ Site location and prior use;
§ Power and water supply;
§ Waste management including sewage, liquid, solid, medical and radioactive waste;
§ Handling and storage of pharmaceutical and chemical products;
§ CFC free equipment;
§ Exposure to medical products and radioactivity;
§ Emergency plans;
§ Health and safety training of employees; and
§ Fire prevention, life safety and emergency response measures.

3. The sponsor has presented plans to address these issues and demonstrate that the proposed project will comply with applicable governmental and World Bank Group requirements. The information provided about how specific impacts are addressed in the development of the project is summarized in the paragraphs that follow.

4. Simmeq is located in Zone 4, a residential area, and the site is surrounded by private houses and a primary school. The building is to be 4 storeys high on a 1,500m2 plot;

5. Water and power are supplied by the local authority and the 301 of fuel to be stored on site will be locked in a cemented room;

6. Run off waters will be collected and disposed off in the Bietry communal collector;

7. Liquid effluents will be treated through 5 septic tanks of 5,000 l capacity each. The tanks are in polyethylene and are provided and emplaced by CORAXEL, a private company specialized in the treatment of sewage and approved by the Ivory Coast Ministry of Construction and Environment. The treatment will comprise a septic tank, a pouzzolanic filter and a soak away. The tank for the kitchen effluents will be further equipped with an oil and grease separator;

8. Solid waste which does not present any risks will be regularly evacuated by the municipal services to the Abidjan landfill;

9. Biomedical and hazardous wastes will be collected separately in specially colored bins or bags and incinerated in situ, under the responsibility of the designated Doctor. The incinerator is to be provided by APICC, a French company specialized in the design and installation of hospital waste incinerators and will have a capacity of 20kg/hour. The total expected quantity of hazardous and biomedical waste to be incinerated is estimated at around 400kg/month and the ashes, about 15kg/month, will be collected in plastic bags to be evacuated with the other solid waste.

10. Radiographic films will be collected by SIMMEG and given to Pharmaciens sans Frontières, a European NGO specialized in the recycling of such waste and which sends it to Europe for recycling in adequate facilities.

11. Quality of liquid effluents and air emissions will be regularly controlled by the local authorities, the periodicity of the controls is to be defined by the inspecting authorities during their first visit and IFC will be informed of their decisions.

12. Storage, handling and disposal of pharmaceutical and chemical products will be carried out in compliance with AFNOR and EN guidelines and placed under the responsibility of either the pharmacist or a doctor;

13. All air conditioning equipment is CFC free;

14. Health and safety issues, including XR protection and regular training of employees is to be addressed in a safety plan to be drafted and implemented by the sponsor and another doctor on staff. A copy of the plan will be send to IFC and it will be implemented at the start of operations.

15. Fire and safety plans :

The main safety issues have been addressed during the building design and construction. It is recommended to conduct a full review of the following features by a professional and registered Engineer, prior to the start up of operations.

Fire load: the presence and control of ignition sources and combustible material inside and outside the property.

Confinement: the passive fire safety features aimed at dividing the premises into compartments, which restrict the spread of fire and smoke. The resistance of the building frame to the effects of fire and smoke is also concerning.

Fire detection and alarm: the capabilities to detect fire and smoke and alerting the occupants and public fire services in case of emergencies.

Escape: The means for safe and accessible exiting of the premise. Topics include the exit system, emergency power, lighting and exit marking.

Fire fighting: the equipment and methods to extinguish fire. It includes manual fire-fighting capabilities and automatic suppression systems.

Emergency Response Plan: the prepared action plan and procedures for overseeing and monitoring the evacuation period. All necessary actions and co-ordination by company staff and local fire brigade should be addressed.

Maintenance: the company management awareness towards fire safety. The integrity of fire protection systems and equipment has to be assured through routine maintenance, inspection, reviews and updates as well as with training of company staff.

16. Local release of the ERS will take place in French, in the Abidjan daily Fraternité Matin and Soir Info and the complete ERS, translated into French will be made available to the public on site

17. Based on its review of available information regarding potential environmental impacts and proposed mitigation measures, IFC concludes that the proposed project is being designed to meet Government of Côte d’Ivoire’s requirements, and World Bank Group policies, and environmental, health and safety guidelines.

18. IFC will monitor SIMMEQ’s ongoing compliance with World Bank Group policies and guidelines during the life of the project by evaluating monitoring reports submitted annually to IFC by the sponsor and by conducting periodic site reviews during project supervision.
